Headway East Northants has recently developed several exciting new posts to provide much needed Outreach Support to the survivors of acquired brain injury (ABI), their families and carers throughout Northamptonshire.

 

These consist of a Family Support Case Manager and a Volunteer Mentor Coordinator, both appointed in November 2009. As a pilot project these workers aim to directly, and through community volunteers, provide support, coaching, motivation, information and advice to brain injury survivors as well as their carers and loved ones.

The Family Support Case Manager provides advice, support and information to the survivors of acquired brain injury (ABI), their families and carers who reside in Northamptonshire.

 

This service is delivered locally, in hospitals, community venues, nursing care and private homes for a period shortly after the injury has occurred The aim is to empower, enable and support clients or family members to adapt to the situation they find themselves in and make informed decisions about their future.

 

The ultimate goals of the service the service are to increase the person’s independence, quality of life and ability to manage in a client centred way. The Family Support Worker will be working to help boosting confidence and motivation, facilitate people to regain skills, and help clients to effectively access local support services.

 

By working with the patient, family, community, health and social care professionals it is hoped that they will achieve a realistic focus on the needs of the client and family to help facilitate a safe, smooth transition from the hospital to rehabilitation or their home.

 

When agreed targets have been met with the family or the clients feel ready to move on from the service appropriate support will be in place including signposting to long term services or other support mechanisms to ensure a supported end to the relationship.
